Many patients ask the same question before thyroid surgery: “Will my voice change?” A short change in the voice after the operation is common and usually settles. Knowing why it happens and what to watch for makes the recovery less stressful.
Why can the voice change after thyroid surgery?
The thyroid gland lies right in front of the voice box (larynx). On each side, a thin nerve called the recurrent laryngeal nerve runs very close to the gland and moves the vocal cords. A second nerve, the superior laryngeal nerve, helps with higher-pitched sounds. During surgery the surgeon identifies and protects these nerves carefully. A voice change can still happen because of:
- The breathing tube used for anaesthesia, which can irritate the throat and vocal cords for a few days,
- Swelling around the neck and voice box after the operation,
- Bruising or stretching of a nerve during the operation, even when the nerve is intact,
- Rarely, an injury to the nerve, which can cause a lasting change.
What is normal after the operation?
- A rough, tired or slightly weak voice and a sore throat for the first days
- Difficulty in singing or speaking loudly, or reaching high notes, for some weeks
- Gradual improvement day by day
Most voice problems after thyroid surgery improve within days to a few weeks, and some take a few months.
Simple voice care at home
- Speak normally and softly. Do not whisper, shout or strain the voice.
- Drink enough water and avoid very cold drinks if they irritate your throat.
- Avoid smoking and smoky or dusty places.
- Take rest from long phone calls, teaching or singing until your surgeon allows it.
When should you see your surgeon?
- The voice is not improving after a few weeks, or gets worse
- Choking or coughing while drinking water or eating
- Breathlessness or noisy breathing
- A very weak voice that does not carry
The surgeon may look at the vocal cords with a small camera (laryngoscopy) to see how they move. If one cord is weak, voice rest, voice therapy or other treatment can help, and the plan depends on what is found.
Can the risk be reduced?
Careful technique, good magnification and an experienced surgeon help to protect the nerves. Please discuss this risk with your surgeon before the operation. It is part of every thyroid surgery consultation, including for endoscopic thyroid surgery. You can meet Dr. Om Prakash Prajapati, endocrine and breast cancer surgeon, at Delta Clinic & Hospital, Varanasi, in person or by video consultation.
